The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Discharge Coordinator, in collaboration with the multidisciplinary clinical treatment team inclusive of nursing and social work, is responsible for the organization of all the patients' discharge planning needs.

Qualifications:

  • Bachelor's Degree In Psychology or other related Human Services field or LPN licensure
  • Minimum 3 years experience in an inpatient psychiatric unit required
  • BSW preferred
  • Training and/or education in an adult psychiatric treatment setting preferred

Scheduling Requirements:

  • Day shift
  • Full Time

Essential Functions:

  • Responsibilities will include but are not limited to implementing and overseeing referral process for next level of care coordination with patient s family support members for meetings and discharge planning.
  • Providing education and resources to patients 1:1 and through group facilitation.
  • Participation in unit based treatment team and run down and arranging addressing transportation and related issues.
  • The Discharge Coordinator takes an active role in hospital functions and activities.
  • All methods of practice shall conform to written hospital policies and procedures and be carried out to the highest achievable level of competence in a manner demonstrating concern and consideration.
  • Is responsible for reporting identified safety issues such as hazardous environments (ie damaged floors walls ceiling tiles unsecured areas and medical errors etc.)
