Short Description The Case Manager for the EMPOWR Program plays a vital role in improving perinatal outcomes for individuals affected by substance use disorders (SUD). This position provides psychosocial assessments, crisis intervention, resource referrals, care coordination, and ongoing supportive services to enhance patient engagement, retention, and overall treatment success. Key Responsibilities Care Coordination & Support Referral, Resource Navigation & Discharge Planning Continuity of Care Collaboration & Professional Development Experience Required 0-2 Years required 3-5 Years preferred Education Requirements Bachelor's Degree Required License/Certification Requirements Certified Social Worker or Licensed Social Worker Preferred Hourly Rate Min $24
Hourly Rate Max $40
