Overview
Manages and supports outreach, education, and care coordination efforts for Veteran patients and families across VNS Health programs, including Home Care, Hospice Care, Behavioral Health, Personal Care, and Health Plans. Supports operational effectiveness, referral development, customer service, and collaboration with internal and external stakeholders to help ensure quality care delivery for Veteran populations. Works under the direction of senior leadership and collaborates closely with interdisciplinary teams.

What You Will Do
- Manages and oversees the day-to-day operations of the Veterans Program, including coordination of staff activities, workflow management, and operational priorities.
- Provides leadership, supervision, coaching, and performance management support to assigned team members; assists with onboarding, training, staff development, and ongoing performance feedback.
- Oversees outreach and educational activities with veteran organizations, community agencies, caregiver coalitions, referral sources, and healthcare partners regarding VNS Health programs and services.
- Implements program strategies and identifies opportunities to strengthen Veteran program growth, referral development, and community engagement.
- Supervises and coordinates program activities to help ensure Veteran patient concerns, benefits coordination, and treatment needs are addressed appropriately.
- Oversees daily staff assignments and assists with issue escalation, operational problem-solving, and prioritization of program activities.
- Supports grant-related activities and assists with monitoring the appropriate utilization of designated funding.
- Develops and maintains collaborative relationships with hospitals, skilled nursing facilities, assisted living facilities, community organizations, and government stakeholders to support referrals and program visibility.
- Oversees coordination of services for complex Veteran patients, including support related to VA benefits navigation and interdisciplinary care planning.
- Manages operational workflows and processes to support timely responses to referral sources, patients, families, physicians, and internal teams.
- Collaborates with internal departments including Hospice, Home Care, Government Affairs, and Business Development to support continuity of care and positive patient outcomes.
- Monitors program compliance with applicable VA, CMS, and NHPCO requirements and supports quality improvement initiatives related to Veteran care services.
- Reviews and analyzes outreach, referral, and operational metrics; prepares reports and provides recommendations to leadership regarding program performance and improvement opportunities.
- Monitors referral trends, conversion activity, and operational performance indicators to support achievement of program goals.
- Represents the program through community outreach, educational presentations, and professional networking activities.
- Participates in special projects and performs other duties as assigned.
Qualifications
Licenses and Certifications:
- Valid driver's license may be required based on operational or regional needs preferred
Work Experience:
- Minimum four years of experience in healthcare, care coordination, community outreach, hospice, home care, or related healthcare services required
- Minimum one year of supervisory, team lead, or program coordination experience preferred
- Experience working with Veteran populations, VA systems, benefits coordination, or related community-based programs required
- Knowledge of the VA healthcare system and Veteran benefits preferred
- Strong oral, written, and interpersonal communication skills required
- Strong customer service, relationship-building, and organizational skills required
- Proficiency with Microsoft Office applications required
- Military service experience or recognized Veteran status with honorable discharge preferred
- Bilingual skills may be required based on operational needs preferred
Pay Range
USD $98,200.00 - USD $130,800.00 /Yr.
