SUMMARY The Community Health Worker (CHW) provides outreach, care coordination, and patient education to help individuals overcome barriers to care and connect with the clinical and community resources they need to thrive. This role supports engagement across key populations, including emergency department, prenatal/postpartum, and post-discharge patients, while promoting continuity of care, self-management, and improved health outcomes. In addition to core CHW responsibilities, this position performs more advanced duties, including mentoring junior staff and contributing to program improvement efforts. QUALIFICATIONS Education
Salary Range: Minimum 24.97/hour - Maximum 41.22/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
