DESCRIPTION OF WORK

As an Aging Care Manager 3, you will help protect older adults by assessing risks, developing care plans, and coordinating needed services. Your work focuses on promoting safety, solving problems, and supporting individuals through careful evaluation and follow-up. In this role, you will perform the following duties:
- Risk Assessment: Gather information from consumers, caregivers, and others to determine the level of risk and whether reports are substantiated
- Care Planning: Develop plans that address concerns, arrange supports, and coordinate needed services to reduce risks
- Case Documentation: Maintain detailed records of contacts, observations, and case activity in the required system
- Home Visits: Conduct face to face visits to monitor safety, review concerns, and speak with individuals involved in the case
- Emergency Response: Arrange shelter, initiate involuntary procedures, or seek guardianship when necessary to protect consumers
- Court Preparation: Prepare documents for court proceedings and provide testimony when required
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
- Travel, overtime, and after hour duties may be required.
- Telework: You may have the opportunity to work from home (telework) part-time, after six months of employment. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Eddystone. The ability to telework is subject to change at any time. Additional details may be provided during the interview.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS Minimum Experience and Training Requirements:
- One year as an Aging Care Manager 2 or Aging Program Assessor; or
- One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.
Other Requirements:
- This particular position also requires possession of a valid driver's license which is not under suspension.

- You must be able to perform essential job functions.
Legal Requirements:
- You must pass a background investigation.
- This position falls under the provisions of the Older Adult Protective Services Act.
- Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
