Elderwood at Burlington is a 150-bed skilled nursing and subacute rehabilitation facility located in beautiful Burlington, Vermont. We provide comprehensive skilled nursing services, long-term care, and rehabilitation programs including orthopedic rehabilitation, speech therapy, and physical therapy. Specialty services include palliative care, respite care, and wound care, allowing us to meet a wide range of resident needs. Social Worker - Position OverviewAs a Social Worker, you will play a vital role in supporting residents and their families by identifying psychosocial, emotional, and mental health needs and connecting them with appropriate resources and services. You will help foster an environment that promotes dignity, independence, individuality, and overall well-being. Working as part of the interdisciplinary care team, you will assist residents through admissions, care planning, transitions of care, and ongoing support throughout their stay.
