FT/Days (40 hours)

The Patient Navigator - Complex Care role supports safe patient and family centered care by meeting the specialized needs of patients and families in MMCP as they progress through their inpatient acute hospital stay. The Clinical Navigator will have the responsibility of enhancing the patient experience, decreasing barriers to care and services, improving quality, and reducing costs for high-risk, technology-dependent patients, while also decreasing fragmentation and duplication of care.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  • Education:BSN required
  • License/Certifications:Maine Licensed RN, willing to become Certified Navigator
  • Experience:Minimum of 3 years in an acute healthcare setting required
  • Additional Skills/Requirements Required:Requires excellent interpersonal skills. Ability to work in a self-directed fast-paced environment working with individuals and leading teams
