The Value-Based Care Pharmacy Technician plays an integral role in advancing the organization's population health and value-based care strategies by improving medication adherence, closing quality care gaps, and enhancing patient outcomes. Working under the supervision of a licensed pharmacist, this position partners with patients, providers, care managers, and payer organizations to identify opportunities for intervention, coordinate medication-related services, and support achievement of quality metrics such as Medicare Star Ratings, HEDIS measures, and other value-based performance indicators. The technician proactively engages patients through outreach, education, medication synchronization, refill coordination, financial assistance programs, and transitions of care initiatives to reduce barriers to medication access and adherence. This role leverages electronic health records, pharmacy management systems, and population health platforms to identify high-risk patients, document interventions, monitor outcomes, and ensure timely follow-up. The Value-Based Care Pharmacy Technician demonstrates a commitment to exceptional patient service, operational excellence, regulatory compliance, and continuous quality improvement while serving as a key contributor to interdisciplinary care teams focused on delivering high-quality, cost-effective, patient-centered care.

Job Description:

Baptist Health is looking for a remote Value Based Care Pharmacy Technician to join our Pharmacy team!
  • Identify patients at risk for medication nonadherence using payer reports, EHR worklists, and pharmacy dispensing data.

  • Conduct outbound patient outreach to improve adherence and address barriers to therapy.

  • Coordinate medication refill reminders, synchronization, and home delivery services.

  • Assist patients with prior authorizations, financial assistance programs, manufacturer copay cards, and insurance navigation.

  • Escalate clinical concerns to pharmacists or providers when medication-related issues are identified.

  • Support transitions of care by ensuring patients obtain discharge medications and understand their medication regimen.

  • Document all patient interactions and interventions in the electronic health record.

  • Monitor performance for Medicare Star Ratings, HEDIS, medication adherence, and other quality measures.

  • Review payer and provider reports to identify opportunities for intervention and gap closure.

  • Collaborate with pharmacists, providers, care managers, and population health teams to coordinate patient care.

  • Track intervention outcomes and assist with reporting on key performance indicators.

  • Participate in outreach campaigns for preventive care and chronic disease management initiatives.

  • Assist with medication reconciliation and ensure accurate medication lists across care settings.

  • Support workflow optimization to improve patient access, adherence, and quality outcomes.

  • Provide exceptional customer service during patient interactions through phone, electronic messaging, and in-person communication.

  • Maintain accurate documentation and comply with regulatory, payer, and organizational requirements.

  • Schedule follow-up calls and manage outreach work queues to ensure timely patient engagement.

  • Assist in developing standardized workflows, educational materials, and process improvements.

  • Participate in quality improvement initiatives and departmental performance meetings.

  • Demonstrate proficiency with pharmacy management systems, electronic health records, and population health technology platforms.

Minimum Requirements

  • High school diploma or equivalent required.

  • Pharmacy Technician registration with Kentucky Board of Pharmacy upon hire.

  • Previous retail, mail order, central/call center, value based care pharmacy work experience preferred.

  • National pharmacy technician certification (PTCB) preferred.
