Schedule/ Work-Life Balance

About the Role Optum HouseCalls is seeking Nurse Practitioners and Physician Assistants to perform annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, field-based role focused on preventive care, patient education, identifying care gaps, and collaborating with primary care providers to improve health outcomes.

Key Responsibilities

  • Conduct comprehensive in-home health assessments
  • Review medical history and medications
  • Perform physical exams, screenings, and point-of-care testing
  • Identify and document diagnoses for care planning
  • Communicate findings to primary care providers
  • Educate patients on disease management and medication compliance
  • Address social determinants of health and connect members with community resources
  • Recognize and respond appropriately to urgent or emergent situations

Required Qualifications

  • NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • PA: NCCPA certification and active state licensure
  • Active, unrestricted licensure (or ability to obtain by start date)
  • Valid driver's license and reliable transportation
  • Ability to perform assessments in various home environments

Preferred Qualifications

  • 1+ years of clinical experience, preferably in family practice, geriatrics, or home health
  • Experience with Medicare or geriatric populations
  • Proficiency with EMR systems and technology
  • Strong communication and patient education skills
  • Ability to transport equipment up to 30 pounds and navigate stairs during home visits

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
