About the Role Optum Senior Community Care is seeking a Nurse Practitioner or Physician Assistant to provide care for high-risk Medicare patients in nursing homes, senior housing, and assisted living communities. The role focuses on comprehensive assessments, chronic disease management, care coordination, and improving quality of life for medically complex patients.

Key Responsibilities

  • Conduct comprehensive patient assessments, including physical exams, medication reconciliation, and screenings
  • Support patients during short-term rehabilitation stays
  • Ensure accurate documentation and ICD-10 coding
  • Collaborate with interdisciplinary care teams
  • Conduct advanced illness and advance care planning discussions
  • Provide education and counseling to patients and caregivers

Required Qualifications

  • NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • PA: NCCPA certification and active state licensure
  • Active, unrestricted state license (or ability to obtain)
  • Current DEA license/prescriptive authority (or ability to obtain)
  • Valid driver's license and reliable transportation
  • Ability to obtain a collaborative practice agreement if required by state law
  • Ability to work in a variety of patient care environments

Preferred Qualifications

  • 1+ years of APC experience, preferably in long-term care
  • Experience in geriatrics, senior living, home care, or long-term care settings
  • Experience caring for patients with complex medical, behavioral, social, or functional needs
  • Knowledge of chronic disease management, geriatrics, advanced illness, and end-of-life care discussions
  • Proficiency with EMR systems and healthcare technology

Compensation for this specialty generally ranges from $53.00 - $79.00 per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
