About the Role Optum Senior Community Care is seeking a Nurse Practitioner or Physician Assistant to provide care for high-risk patients in assisted living and senior living communities in the Salem area. This role combines elements of geriatrics, internal medicine, chronic disease management, and palliative care within a value-based care model.

Key Responsibilities

  • Conduct comprehensive patient assessments
  • Manage acute illnesses and chronic disease exacerbations
  • Coordinate testing, diagnostics, and treatment interventions
  • Ensure accurate documentation and ICD-10 coding
  • Collaborate with interdisciplinary care teams
  • Conduct advanced illness and advance care planning discussions
  • Provide education and counseling to patients and caregivers
  • Coordinate care transitions and develop individualized care plans

Required Qualifications

  • NP: MSN or DNP degree with AANP or ANCC certification
  • PA: Graduate of an accredited Physician Assistant program with NCCPA certification
  • Active state license (or ability to obtain)
  • Active DEA license or ability to obtain
  • Valid driver's license and reliable transportation
  • Ability to perform home and facility-based assessments
  • Availability to work at least 24 hours per month

Preferred Qualifications

  • New graduates encouraged to apply
  • Experience in geriatrics, senior living, home care, or chronic disease management
  • Experience caring for medically and socially complex patients
  • Knowledge of advanced illness and end-of-life care discussions
  • EMR proficiency and strong documentation skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $43.22 to $77.21 per hour based on full-time employment. We comply with all minimum wage laws as applicable
