This role combines elements of geriatrics, internal medicine, chronic disease management, and palliative care within a value-based care model. Key Responsibilities Conduct comprehensive patient assessments Manage acute illnesses and chronic disease exacerbations Coordinate testing, diagnostics, and treatment interventions Ensure accurate documentation and ICD-10 coding Collaborate with interdisciplinary care teams Conduct advanced illness and advance care planning discussions Provide education and counseling to patients and caregivers Coordinate care transitions and develop individualized care plans Required Qualifications NP: MSN or DNP degree with AANP or ANCC certification PA: Graduate of an accredited Physician Assistant program with NCCPA certification Active state license (or ability to obtain) Active DEA license or ability to obtain Valid driver's license and reliable transportation Ability to perform home and facility-based assessments Availability to work at least 24 hours per month Preferred Qualifications New graduates encouraged to apply Experience in geriatrics, senior living, home care, or chronic disease management Experience caring for medically and socially complex patients Knowledge of advanced illness and end-of-life care discussions EMR proficiency and strong documentation skills Highlights Value-based care focused on outcomes rather than productivity Manageable patient volume of approximately 4-6 visits per day Strong interdisciplinary team support Opportunity to build meaningful relationships with seniors in assisted living and community-based settings Excellent work-life balance and flexibility Ideal for clinicians interested in geriatrics, palliative care, and care coordination. The hourly pay for this role will range from $43.22 to $77.21 per hour based on full-time employment.
