About the Role Optum Senior Community Care is seeking a Nurse Practitioner or Physician Assistant to provide care for high-risk patients in assisted living and senior living communities in the Salem area. This role combines elements of geriatrics, internal medicine, chronic disease management, and palliative care within a value-based care model. Key Responsibilities

The hourly pay for this role will range from $43.22 to $77.21 per hour based on full-time employment.
