About the Role Optum Senior Community Care is seeking a Nurse Practitioner or Physician Assistant to care for high-risk patients in assisted living communities, senior housing, and nursing home settings. This value-based care role focuses on quality outcomes, chronic disease management, palliative care principles, and building meaningful patient relationships. Providers work as part of an interdisciplinary team caring for medically complex senior populations.

Key Responsibilities

  • Conduct comprehensive patient assessments
  • Manage acute illnesses and chronic disease exacerbations
  • Coordinate diagnostics, testing, and treatment interventions
  • Ensure accurate documentation and ICD-10 coding
  • Collaborate with facility staff, families, specialists, and care teams
  • Conduct advanced illness and advance care planning conversations
  • Educate patients and caregivers
  • Coordinate care transitions and develop individualized care plans

Required Qualifications

  • NP: MSN or DNP degree with AANP or ANCC certification
  • PA: Graduate of an accredited Physician Assistant program with NCCPA certification
  • Active state license (or ability to obtain)
  • Active DEA license or ability to obtain
  • Ability to obtain a collaborative practice agreement if required
  • Reliable transportation
  • Ability to perform physical assessments and travel between care settings

Preferred Qualifications

  • New graduates encouraged to apply
  • Experience in senior living, geriatrics, home care, or chronic disease management
  • Experience caring for medically complex patients
  • Understanding of geriatrics, chronic illness, advanced illness, and end-of-life care
  • EMR proficiency and strong documentation skills

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
