In Home Care, No Nights, Weekends or Holidays

This is a full time position where you can work 5 - (8 hour days) or 4 - (10 hour days)!

About the Role Optum Care at Home is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal, home-based care for high-risk patients with complex medical needs. Providers are part of an interdisciplinary care team focused on helping patients remain safely at home, improving quality of life, and managing chronic and acute health conditions.

Key Responsibilities

  • Conduct comprehensive in-home assessments, including physical exams, medication reviews, screenings, and diagnostics
  • Manage acute illnesses and chronic disease exacerbations
  • Support patients during short-term rehabilitation episodes
  • Provide advanced care planning and end-of-life discussions
  • Educate patients and caregivers
  • Ensure accurate ICD-10 documentation and coding
  • Collaborate closely with interdisciplinary care teams and other providers

Required Qualifications

  • NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • PA: NCCPA certification and active state licensure
  • Active, unrestricted license (or ability to obtain)
  • Active DEA license/prescriptive authority (or ability to obtain)
  • Valid driver's license and reliable transportation
  • Ability to work independently in various home-based care settings

Preferred Qualifications

  • 1+ years of NP/PA clinical experience
  • Experience in geriatrics, home health, senior living, or long-term care
  • Experience caring for medically complex patients
  • Knowledge of chronic illness, advanced illness, and end-of-life care
  • EMR and technology proficiency

Compensation & Benefits

  • Salary range generally $109,500-$164,000, plus bonus opportunities

Compensation for this specialty generally ranges from $109,500 to $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
