This position requires daily travel to patient homes within assigned area.

About the Role Optum Care at Home is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal, in-home care for high-risk patients. Providers work as part of an interdisciplinary care team managing complex chronic conditions, acute medical needs, and care transitions in the patient's home environment.

Key Responsibilities

• Conduct comprehensive in-home assessments, including physical exams, medication reviews, and screenings
• Manage acute illnesses and chronic disease exacerbations
• Support patients during short-term rehabilitation episodes
• Provide advanced care planning and end-of-life discussions
• Educate patients and caregivers
• Ensure accurate ICD-10 documentation and coding
• Collaborate closely with interdisciplinary care teams

Required Qualifications

• NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• PA: NCCPA certification and active state licensure
• Active, unrestricted license (or ability to obtain)
• Active DEA license/prescriptive authority (or ability to obtain)
• Valid driver's license and reliable transportation
• Ability to work independently in a variety of home-based settings

Preferred Qualifications

• 1+ years of clinical experience as an NP or PA
• Experience in geriatrics, home health, senior living, or long-term care
• Experience caring for medically complex patients
• Knowledge of chronic illness, advanced illness, and end-of-life care
• EMR and technology proficiency

Compensation & Benefits

• Compensation generally ranges from $109,500 - $164,000, plus bonus opportunities

Compensation for this specialty generally ranges from $109,500 to $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
