Part-time Nurse Practitioner or Physician Assistant, SNF Patient Connect - Multnomah and Yamhill Counties, OR

About the Role This role is part of Optum's SNF Patient Connect Program, which supports UnitedHealthcare members in short-term rehabilitation and transitional care settings. Providers complete a one-time comprehensive health assessment and collaborate with case managers, facility staff, and primary care providers to improve care coordination, reduce hospital readmissions, close quality gaps, and support successful transitions of care.

Key Responsibilities

  • Conduct comprehensive health assessments for patients in skilled nursing facilities and senior living settings
  • Collaborate with primary care physicians, nursing staff, families, and case management teams
  • Support care coordination and discharge planning
  • Complete documentation, coding, and gap closure activities
  • Help improve quality measures, medical record accuracy, and patient outcomes

Required Qualifications

  • NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • PA: NCCPA certification and active state licensure
  • Active, unrestricted state license (or ability to obtain)
  • DEA licensure/prescriptive authority (or ability to obtain)
  • Valid driver's license and reliable transportation
  • Ability to obtain a collaborative practice agreement if required
  • Ability to work in various patient care settings and perform physical assessments

Preferred Qualifications

  • 1+ years of APC experience, preferably in long-term care
  • Experience in geriatrics, senior living, home care, or skilled nursing settings
  • Experience caring for patients with complex medical, behavioral, or functional needs
  • Knowledge of chronic illness, geriatrics, advanced illness, and end-of-life care discussions
  • EMR and healthcare technology proficiency

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
