About the Role As an Advanced Practice Clinician (NP or PA) with Optum's Senior Community Care program, you will provide comprehensive care to high-risk patients residing in skilled nursing facilities, assisted living communities, and senior housing settings. The role focuses on improving health outcomes, reducing unnecessary hospitalizations, supporting post-acute rehabilitation, and helping patients navigate complex medical needs. Key Responsibilities

Compensation for this specialty generally ranges from $53.00 - $79.00 per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
