About the Role As an Advanced Practice Clinician (NP or PA) with Optum's Senior Community Care program, you will provide comprehensive care to high-risk patients residing in skilled nursing facilities, assisted living communities, and senior housing settings. The role focuses on improving health outcomes, reducing unnecessary hospitalizations, supporting post-acute rehabilitation, and helping patients navigate complex medical needs.

Key Responsibilities

• Perform comprehensive patient assessments and physical exams
• Review medical histories, medications, and care plans
• Support patients during short-term rehabilitation stays
• Document diagnoses and clinical findings accurately
• Collaborate closely with physicians, nurses, social workers, and care teams
• Conduct advance care planning and serious illness conversations
• Educate patients and caregivers on disease management and treatment plans
• Help coordinate care for patients with complex medical, behavioral, and social needs

Required Qualifications

• NP: Active AANP or ANCC certification
• PA: Active NCCPA certification
• Active, unrestricted state license (or ability to obtain)
• Valid driver's license and reliable transportation
• Ability to obtain collaborative practice agreement if required by state regulations

Preferred Qualifications

• 1+ years of NP/PA experience
• Experience in geriatrics, long-term care, senior living, home care, or primary care
• Knowledge of chronic disease management and complex care populations
• Experience with end-of-life and advance care planning discussions
• Strong EMR and technology skills

Compensation for this specialty generally ranges from $53.00 - $79.00 per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
