bout the Role

As a HouseCalls Advanced Practice Clinician (NP or PA), you'll conduct annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, preventive care role focused on identifying health risks, closing care gaps, educating patients, and collaborating with primary care providers to improve overall health outcomes.

Key Responsibilities
  • Conduct comprehensive in-home health assessments
  • Review medical history, medications, vital signs, and physical exams
  • Perform evidence-based screenings and point-of-care testing as appropriate
  • Identify and document diagnoses and care needs
  • Communicate assessment findings to primary care providers
  • Educate members on disease management, medications, and preventive care
  • Address social determinants of health and connect members with resources
  • Recognize urgent health concerns and facilitate appropriate follow-up
Qualifications

Required

  • NP: Active ANCC or AANP certification
  • PA: Active NCCPA certification
  • Active, unrestricted state license (or ability to obtain)
  • Valid driver's license and reliable transportation
  • Ability to travel to patients' homes and perform assessments in varied environments

Preferred

  • 1+ years of clinical experience
  • Family practice, geriatrics, home health, or primary care experience
  • Experience working with Medicare populations
  • Strong EMR and technology proficiency
  • Excellent communication and patient education skills
Ideal Candidate

This role is ideal for an NP or PA seeking schedule flexibility, autonomy, meaningful patient interactions, and a focus on preventive care while helping patients remain healthier and independent in their homes.

Compensation for this specialty generally ranges from $109,500 to $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
