Per diem NP or PA, Long Term Care, Lane County, OR Incentives & Schedule $5,000 Sign-On Bonus for external candidates Value-based care model with no productivity requirements Flexible schedule with a minimum commitment of 24 hours per month No routine on-call, weekends, or holidays required Approximately 4-6 patient visits per day, allowing for longer, more meaningful patient interactions About the Role Optum Senior Community Care is seeking a Nurse Practitioner or Physician Assistant to provide care to high-risk patients in senior living communities, assisted living facilities, and other community-based settings. This role combines geriatric and palliative care principles with a strong focus on quality outcomes, care coordination, and relationship-based medicine. Key Responsibilities Conduct comprehensive patient assessments Manage acute illnesses and chronic disease exacerbations Order and interpret diagnostics and interventions as appropriate Ensure accurate documentation and ICD-10 coding Collaborate with interdisciplinary care teams, facility staff, families, and physicians Conduct advanced illness and advance care planning conversations Educate patients and caregivers Coordinate care as patients transition between care settings Develop and implement patient-centered care plans Required Qualifications NP: Graduate of an accredited MSN or DNP program with board certification (AANP or ANCC) PA: Graduate of an accredited Physician Assistant program with NCCPA certification Active Oregon license (or ability to obtain) DEA license/prescriptive authority (or ability to obtain) Ability to obtain a collaborative practice agreement if required Reliable transportation Ability to perform home and facility-based assessments and transport equipment Preferred Qualifications New graduates encouraged to apply Experience in senior living, geriatrics, home care, or chronic disease management Experience caring for medically complex patients Knowledge of geriatrics, chronic illness, palliative care, and end-of-life discussions EMR proficiency and strong documentation skills Benefits Competitive salary plus incentive opportunities Comprehensive medical, dental, and vision coverage Retirement plans with employer contributions Employee stock purchase plan Paid time off Continuing education support Paid parental leave Mileage reimbursement Equipment stipend Extensive clinician development and training programs Highlights This role offers a value-based care model that prioritizes quality over volume, allowing providers to spend more time with patients, build meaningful relationships, and make a significant impact on the health and well-being of seniors in community-based settings.
