About the Role Optum Senior Community Care is seeking a Nurse Practitioner or Physician Assistant to provide care to high-risk patients in senior living communities, assisted living facilities, and other community-based settings. This role combines geriatric and palliative care principles with a strong focus on quality outcomes, care coordination, and relationship-based medicine. Key Responsibilities
