About the Role Optum Senior Community Care (SCC) is seeking a Nurse Practitioner or Physician Assistant to provide ongoing care for high-risk patients in nursing homes, assisted living communities, and senior housing settings. Providers work as part of an interdisciplinary team delivering longitudinal, facility-based care focused on improving outcomes for medically complex seniors.

Key Responsibilities

  • Conduct comprehensive patient assessments
  • Manage acute illnesses and chronic disease exacerbations
  • Support patients during short-term rehabilitation stays
  • Ensure accurate documentation and ICD-10 coding
  • Collaborate with interdisciplinary care teams
  • Conduct advanced illness and advance care planning discussions
  • Educate and counsel patients and caregivers
  • Coordinate care and treatment planning across care settings

Required Qualifications

  • NP: MSN or DNP degree with AANP or ANCC certification
  • PA: Graduate of an accredited Physician Assistant program with NCCPA certification
  • Active, unrestricted state license (or ability to obtain)
  • DEA license/prescriptive authority (or ability to obtain)
  • Ability to obtain a collaborative practice agreement if required
  • Valid driver's license and reliable transportation
  • Ability to perform physical assessments and travel between patient care locations

Preferred Qualifications

  • 1+ years of clinical experience, preferably in long-term care, geriatrics, or senior living
  • Experience caring for medically complex patients
  • Knowledge of geriatrics, chronic illness, and end-of-life care discussions
  • EMR proficiency and strong documentation skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $89,900 to $160,600 annually based on full-time employment. We comply with all minimum wage laws as applicable.
