Incentives & Schedule

About the Role Optum Senior Community Care is seeking a Nurse Practitioner or Physician Assistant to provide care to high-risk patients in senior living communities, assisted living facilities, and other community-based settings. This role combines geriatric and palliative care principles with a strong focus on quality outcomes, care coordination, and relationship-based medicine.

Key Responsibilities

  • Conduct comprehensive patient assessments
  • Manage acute illnesses and chronic disease exacerbations
  • Order and interpret diagnostics and interventions as appropriate
  • Ensure accurate documentation and ICD-10 coding
  • Collaborate with interdisciplinary care teams, facility staff, families, and physicians
  • Conduct advanced illness and advance care planning conversations
  • Educate patients and caregivers
  • Coordinate care as patients transition between care settings
  • Develop and implement patient-centered care plans

Required Qualifications

  • NP: Graduate of an accredited MSN or DNP program with board certification (AANP or ANCC)
  • PA: Graduate of an accredited Physician Assistant program with NCCPA certification
  • Active Oregon license (or ability to obtain)
  • DEA license/prescriptive authority (or ability to obtain)
  • Ability to obtain a collaborative practice agreement if required
  • Reliable transportation
  • Ability to perform home and facility-based assessments and transport equipment

Preferred Qualifications

  • New graduates encouraged to apply
  • Experience in senior living, geriatrics, home care, or chronic disease management
  • Experience caring for medically complex patients
  • Knowledge of geriatrics, chronic illness, palliative care, and end-of-life discussions
  • EMR proficiency and strong documentation skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $44.09 to $78.70 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
