About the Role Optum Home-Based Medical Care is seeking a Nurse Practitioner or Physician Assistant to provide in-home care for medically complex, high-risk patients. This role focuses on helping patients remain safely at home, reducing hospitalizations and ER visits, improving quality outcomes, and managing chronic illnesses through longitudinal care. Ideal Candidate An NP or PA who enjoys autonomy, chronic disease management, geriatrics, home-based care, and developing long-term relationships with complex patients while helping them avoid unnecessary hospitalizations and remain safely at home. Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
