The Optum Care at Home (OAH) program delivers longitudinal, integrated care to high-risk patients in the comfort of their homes. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you'll serve as a key member of an interdisciplinary care team, providing comprehensive assessments, managing acute and chronic conditions, and partnering with patients and caregivers throughout their healthcare journey.

This role offers the opportunity to practice autonomously, build lasting patient relationships, and make a direct impact on quality of life while caring for some of our most vulnerable populations.

• Perform comprehensive in-home assessments, including:
  • Medical history review
  • Medication reconciliation
  • Physical examinations and vital signs
  • Evidence-based screenings and point-of-care testing, when appropriate
• Manage patients experiencing acute illnesses or exacerbations of chronic conditions through assessment, diagnostics, and treatment interventions
• Support members during short-term rehabilitation and recovery periods
• Maintain accurate and timely clinical documentation, including ICD-10 coding
• Collaborate closely with physicians, nurses, care coordinators, and other interdisciplinary team members
• Lead advanced illness and goals-of-care discussions with patients and families
• Provide education, counseling, and support to patients and caregivers
