What You Will Do: In this exciting newly created role, the Assistant Case Manager will play a key leadership role in supporting and shaping the success of our case management team across multiple sites. You will supervise daily operations, help establish a strong foundation for the team, and contribute to the delivery of efficient, effective, and patient-centered care. You will coordinate nursing case management activities, collaborate with interdisciplinary healthcare professionals to optimize patient outcomes, and ensure nursing practice remains aligned with organizational policies and healthcare regulations.
