DESCRIPTION

Under the supervision of the medical director and the NP, the Cystic Fibrosis Clinic Nurse coordinates outpatient clinic care for patients with Cystic Fibrosis (CF) and other complex pulmonary conditions. The CF program is an accredited care center that provides comprehensive, high quality-care to cystic fibrosis patients.

The CF Clinic Nurse will be working with the multidisciplinary CF care team including social workers, dietitian, respiratory therapist, pharmacist, and administrative staff. The CF Clinic Nurse collaborates with physicians and multidisciplinary care team to provide access to clinical care and advice outside of the clinic setting. The CF nurse also coordinates with outside clinicians, healthcare teams, services as the key liaison between patients, caregivers, and healthcare team, promoting continuity of care and optimal patient outcomes

Key areas of responsibility include direct patient care, patient and family education regarding disease processes and treatments, and coordination of CF program activities. Specific duties include triage (telephone, MyChart, and in-person), assisting with provider inbox coverage, coordinating outpatient intravenous therapy (IV) therapy, performing clinic procedures (i.e.: six-minute walk and O2 titration), ensuring access to routine and specialty medications, organizing patient education and support activities, facilitating multidisciplinary care coordination, and maintaining clinic-specific reports and quality metrics.

Autonomous nursing care is the ability of a nurse to assess and provide nursing interventions as appropriate for patient populations based on competence, professional expertise, and clinical knowledge. The nurse is expected to practice autonomously, consistent with ANA Standards of Practice and Professional Performance. Independent judgment is exercised within the context of interdisciplinary and multidisciplinary approaches to patient care. All performance expectations associated with this position are performed at the PRACTITIONER level.

MINIMUM QUALIFICATIONS
  • Must be a graduate from an accredited school of Nursing.

  • RN License issued by the State of California.

  • BART or BLS at time of hire with commitment to get BART within six (6) months of hire date.

  • Minimum of 2 years of recent nursing experience in ambulatory or acute care/hospital setting within the past 5 years.

  • Must be able to perform pulmonary clinic procedures (i.e: six minute walk, O2 titration).

  • Strong critical thinking and problem-solving skills to manage multiple levels of information and responsibilities, and to quickly assess moderately-complex problems to develop potential solutions.

  • Must be able to function in a fast-paced environment, have excellent interpersonal and communication skills, and consistently demonstrate empathy and kindness to all staff and patients.

  • Demonstrates critical thinking and clinical skills to identify and implement nursing interventions and evaluate results.

  • Must have excellent skills to communicate and influence effectively with all levels of staff, physicians, patients and external constituents, both verbally and in writing.

PREFERRED QUALIFICATIONS
  • Two (2) years experience working with Chronic Pulmonary Disease such as Cystic Fibrosis.
  • BSN.
  • ACLS and/or ART certification.
  • Previously direct patient care experience in a healthcare setting, preferably in chronic or serious illness such as Cystic Fibrosis.
  • Experience in a Pulmonary Setting.
SPECIAL CONDITIONS
  • Monday to Friday 8 hr. day shifts (Thursdays 8 hr. shift - early evening - clinic from 5pm to 9pm)
  • Must be able to work various hours, days, shifts, on-call and various locations based on the 24-hour Medical Center's business needs.
  • Employment is subject to a criminal background check and pre-employment physical.
  • Selected candidate may be hired at the CNIII level pending review and approval by the Nurse Executive Council.

Pay Transparency Act

Annual Full Pay Range: $138,330 - $193,892 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $66.25 - $92.86

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
