MAKE IT HAPPEN

This position is grant-funded for a period of five years and is therefore considered temporary in nature. While the role is expected to continue for the duration of the grant, ongoing employment is contingent upon continued funding.

Position Summary

Engages with community members to identify health and social risk factors, ensuring they receive the necessary resources and support. Coordinates and facilitates communication between clients and healthcare teams to promote prenatal and postpartum health. Provides guidance to clients on available programs and benefits, and assists with overcoming barriers to care, such as transportation and appointment scheduling. Provides training to junior team members on procedures and required processes.

Key Responsibilities

* Evaluates members' health and social risk factors, coordinates plans with the healthcare team for identified needs, and connects members with suitable community resources.

* Completes documentation to ensure all interactions and referrals are recorded accurately.

* Coordinates care and collaborates with the healthcare team, communicates member risks and needs, and manages changes in member status.

* Educates members about programs and benefits, utilizing both internal and external resources to help them find appropriate benefits.

* Prioritizes outreach activities based on member needs and risk levels, responds to referrals, and provides feedback to external referral sources within specified timeframes.

* Assists enrollees with transportation barriers for prenatal and postpartum appointments, screens transportation requests, and coordinates with local transportation assistance.

* Engages members in care through phone contacts, evaluates and resolves inquiries, and documents all efforts using appropriate procedures.

* Supports special projects, tracks program progress, and completes all requirements as specified and on time.

* Identifies appropriate patients for examinations and coordinates with the healthcare team to improve care coordination and make suitable referrals to community resources.

* Maintains relationships with referral and community resources to ensure seamless collaboration and support for members' needs.

* Guides and advocates for members, helping them navigate the health system and schedule appointments as needed.

Required Qualifications

• High School Diploma/GED or equivalent (Required)
• 2+ years of experience in community health, social services, or a related field (Required)
• Navigate rapidly changing situations, from evolving patient needs to technological advancements, by remaining flexible, continuously learning, embracing new challenges, and quickly recovering from setbacks.
• Ability to read and communicate effectively in English. Strong written and verbal skills.
• Completes basic to moderately complex clinical support assignments; work involves minimal problem resolution.
• Strong attention to detail and self-directed to consistently ensure data integrity and accuracy.
• Uphold ethical principles by maintaining confidentiality, ensuring informed consent, and making decisions that prioritize the well-being of both patients and staff.
• Work seamlessly within diverse teams, bringing together professionals from various disciplines to provide patient-centered care and achieve collective goals.
• Follows established protocols and guidelines ensuring safety of patient information and hospital/clinical assets.
• Makes decisions that are guided by general instructions and practices requiring some interpretation.
• Solves basic to moderately complex clinical support challenges.
• Applies experience and working knowledge to perform patient-facing, but indirect patient care activities in Health Programs.
• Executes tasks with attention to cost-efficiency and high productivity.
• Proficient computer skills. Ability to use an electronic medical record system may be required in certain areas.
