Summary:
To provide comprehensive clinical services as part of a multi-disciplinary team assessment crisis intervention behavioral skill training case management education and counseling services to clients and to provide clinical consultation services to the team in order to assist clients in achieving treatment and rehabilitation goals. To enhance the quality of life of adults with persistent mental illness through recovery strategies.

Responsibilities:
MAIN DUTIES AND RESPONSIBILITIES:

Provide individual brief treatment and support to clients and their families in order to assist in developing coping skills for behavioral management and controlling psychiatric symptoms as prescribed in the treatment plan. Facilitate support group sessions as assigned.

Function as the primary worker for a caseload of clients as assigned by the Program Manager. This responsibility includes Intake assessments skills training resource linkage attending medication evaluation appointments and psychiatric evaluations treatment planning documentation of recovery and progress and any other duty as indicated by a comprehensive assessment of needs for each client and maintaining the clinical record with all necessary paperwork according to accreditation funding source agency and program standards and policies. Maintaining entitlement benefits for clients and insurance authorizations for services.

Follow agency policies and standards for client rights and confidentiality.

Participate as a member of a multi-disciplinary team in the development and implementation of treatment plans. This includes but is not limited to: completing assessment assignments within time requirements specified by procedures scheduling/attending treatment planning meetings with staff and other individuals specified in procedures utilizes assessment information diagnostic results and staff reports in the development of treatment plans.

Attend biweekly or as needed supervision meetings weekly team meetings and other activities as required. .

Maintain a valid driver*s license auto insurance and the use of a personal vehicle in order to conduct community home and hospital visits as well as to transport clients as needed.

Maintain annual CPR certification and any other related certifications / licenses as required or as needed.

Maintain all mandated agency training certifications as required.

Maintain current knowledge of Agency P&P implement in daily routine and follow all Health Home procedures as indicated.

Serves on agency committees and task forces as appropriate and as assigned.

In addition to listed primary Clinical Specialist responsibilities the Clinical Specialist will provide assistance with and facilitation of psychiatric rehabilitation related assessment(s) interventions and services that will facilitate the client*s achievement of their Recovery and Overall Rehabilitation Goals as required by and indicated on the Client*s Master Treatment Plan. When assessing planning for and providing Health Home services the individual along with assistance and consultation from the Health Home Rehabilitation Specialist and with supervision from team leadership must maintain a strong focus and prioritize Rehabilitative related service activities in order to maximize the client*s functional outcome(s) achievement with chosen roles within chosen environments and overall potential for maximum recovery. Some of the additional duties and responsibilities required to fulfill this role are but not limited to:

Utilize Stages of Change theory Motivational Enhancement techniques and Rehabilitation Goal Readiness & Development Technology & Process to facilitate maximum potential for rehabilitation goal Attainment.

Become familiar with and provide consultation on client subsidy work incentives and program such as the impact of work on SSDI SSI Medicaid and Hosing Subsidies.

Provide assistance to clients as needed with the completion of documentation administrative tasks and processes related to the client obtaining employment accessing educational programs and other community organization(s) related to their rehabilitation goal(s) such as completing Job Applications(s) financial aid packages etc.

Assist client(s) and rehabilitative specialist(s) with required job educational and social/leisure development services and activities as needed.

Assist team Rehabilitative Specialist with providing on/off site supports assessed as needed on order to facilitate client job and/or client meaningful activity retention satisfaction and success.

Provide interventions and services designed to assist the client with obtaining satisfying social leisure opportunities and community activities that maximize the cline*s level of community integration as well as assist the cline in obtaining and maintaining a recovery promoting natural support system and peer relationships.

Other duties as assigned.

QUALIFICATION REQUIREMENTS:

Masters* degree in any Human Services field required such as Social Work Rehabilitation or Agency Counseling with three years experience working with adults with serious mental illness plus experience in assessment counseling crisis intervention substance abuse and sexual abuse.

NECESSARY SPECIAL REQUIREMENTS:

Maintain valid driver*s license at all times.

Ability to work evenings on a rotating basis as needed.

Ability to communicate with clients and other staff face to face and by telephone.

Ability to obtain and maintain C.P.R. certification.

Ability to work as a multi disciplinary team member ability to accept establish and maintain working relationships with diverse populations of clients and staff ability to maintain objectivity and ability to maintain perspective under stress; resilience.

Maintain valid driver*s license at all times.

Ability to work evenings on a rotating basis as needed.

JOB KNOWLEDGE SKILL AND ABILITY:

Ability to read analyze and interpret professional journals and governmental regulations.

Ability to write reports and business correspondence.

Ability to effectively present information and respond to questions from groups of managers clients customers and the general public.

Knowledge of and/or willingness to develop computer literacy skills.

Ability to successfully complete all required training and certifications

Oral and written communication skills including speech grammar spelling and legibility are at sufficient level to ensure effective and appropriate communication.

Ability to add subtract multiply and divide in all units of measure using whole numbers common fractions and decimals either manually or by use of a calculator.

Uses and accesses information in client*s health information record or about a client only as necessary to actual duties and then only in accordance with access levels assigned to this position by Gateway Healthcare Inc. from time-to-time and in accordance with Gateway Healthcare Inc. policies rules and procedures as adopted by Gateway Healthcare Inc. from time-to-time.

Must be able to appreciate the sensitive/private nature and legal status of information about individual clients obtained maintained or used by Gateway Healthcare Inc. and to not access use disclose or request such information about a client unless necessary to the performance of his/her job.
