Support care management staff with delivery of service to clients. Act as client advocate to support with health system navigation and education.

RESPONSIBILITIES

  • Assist Care Managers with supporting clients with meeting care plan goals by breaking down barriers to accessing goods and services.
  • Establishes and maintains professional relationships with clients and their informal support network in a culturally appropriate manner, offering respect and dignity in all interactions.
  • Provide resource materials to the clients served to include helping coordinate health and medical appointments and assist in the referral of community resources and activities to support client health/wellness and independence in the community.
  • Assist Care Managers in completing necessary documentation to enroll and maintain Medi-Cal Waiver participation for clients.
  • Ensures receipt or delivery of purchased/covered goods and services to clients.
  • Escorts clients to appointments and other necessary community activities as needed.
  • Assist clients with navigating IHSS Public Authority and interviewing for IHSS providers to fill gaps in home/personal care for clients living in independent or senior housing as needed.
  • Maintain accurate record of client service in electronic database.
  • Attends and actively participates in team and program meetings, activities and problem-solving endeavors; contributes to open lines of communication within the team.
  • Actively incorporates the ethical and legal standards of the National Association of Social Workers into all aspects of interactions with others.
  • Understands and applies the regulatory and procedural requirements of the Institute on Aging.
  • Attends continuing education classes and/or inservice training to increase knowledge, skills and attitudes related to case management, gerontology, family and community systems and other areas relevant to the client population.
  • Other duties as assigned.

BACKGROUND AND EXPERIENCE:

  • One year's experience working within a community-based case management program or with a similar target population required.
  • Demonstrated understanding and interest in working with older adults or adults with physical, cognitive or behavioral health (including substance use) impairments.
  • Experience with home visiting preferred.
  • Detail-oriented with good problem-solving, time management, and communication skills.
  • Computer literacy required.

EDUCATION: High school diploma or equivalent required. Some college/degree in human services related field preferred.

COMPENSATION:

Range: $ 25.15 - 31.60/hour

This amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education and other factors.

This range does not include any additional equity, benefits, or other non-monetary compensation which may be included.
