Wages and Benefits

$23 - $25 per hour; paid vacation time, sick time, plus nine paid holidays and three floating holidays; competitive health, dental, and vision insurance; Health Savings Account (HSA), and options for Flexible Spending Accounts (FSA). There is employer paid basic life insurance with the option for additional coverage paid by the employee. The employer pays for Accidental Death and Dismemberment (AD&D) and short- and long-term disability insurance. FWS sponsors a retirement plan with 3% employer match and immediate vesting.

Schedule

This position works during normal business hours and may include evening and/or weekend hours. This position also requires travel to and from community settings, schools, as well as shelters and the homes of families. This position may include transporting children and families. This role is hybrid with required in-office time, required time in the community, and the option to work from home when approved by supervisor.

A Typical Day in the Life...

A typical day as FamilyWise's Wraparound Case Manager provides an opportunity to facilitate a team process to give families voice and choice, with the goal of helping young people with behavioral health issues to live in a stable family or community setting. Responsibilities will include the following:



  • Engaging children and families involved with multiple systems.
  • Developing and facilitating family-centered teams.
  • Conducting strengths and needs assessments.
  • Developing and implementing individualized Wraparound plans.
  • Collaborating with community partners.
  • Modeling appropriate social skills for parents and their children.
  • Communicating effectively with county workers.
  • Supporting parents to improve life skills to be able to manage new needs as they arise.
  • Participating in training, credentialing, and evaluation processes for High Fidelity Wraparound.
  • Participating in monthly group coaching meetings and case consultations.
  • Participating in other trainings as applicable.
  • Maintaining client files to include assessments, program plans, client progress, and assessments by other professionals, intake forms, client outcome data, referrals, and other pertinent information.


Who You Are



  • You have a bachelor's degree in social work, psychology, parent education, criminal justice, or a related field, or you can demonstrate equivalent knowledge gained through alternative courses of study and life experience.
  • You have knowledge and experience working with culturally and racially diverse populations in lower socio-economic conditions.
  • You can speak multiple languages, which is highly valued.
  • You demonstrate strong organizational skills, allowing you to manage multiple priorities and meet deadlines.
  • You understand the importance of customer service and professionalism.
  • You showcase skills in conflict management.
  • You are computer literate, with proficiency in MS Office.
  • You can work both independently and as part of a team.
  • You use good judgment in recognizing the scope of your authority.
  • You can travel to client sites within a 15-mile radius of the office.
  • You have a valid driver's license, access to reliable transportation, and the ability to meet FamilyWise's driving record requirements. Please note that if the license was not issued in the state of Minnesota, you have 60 days after becoming a Minnesota resident in which to apply for your Minnesota driver's license or permit.
