SUMMARY OF POSITION: As a social worker will work as part of an interdisciplinary team in the Moses Campus - Inpatient Med/Surg units on the weekends. Will provide a range of direct and indirect services for patients and families, including, but not limited to discharge planning and the identification of psychosocial problems and their resolutions. Will collaborate with patients, families, and team members to ensure safe and timely discharges and treatment plans. REQUIREMENTS AND EDUCATION: Masters in Social Work from a school of Social Work from a program registered by the NYS Dept of Education or determined to be substantially equivalent by the commissioner's regulations. Current New York LMSW or LCSW is required, if on Limited Permit, they must havean executed Memorandum of Understanding with a licensed Social Worker for the purposes of supervision and must obtain LMSW within 6 months of employment.
