Job Description The Therapist has the responsibility of providing clinical services that are office and/or home based to consumers of all ages in jurisdictions served by our company. Services are provided with client-centered and family-centered approaches and are derived through time-limited, goal specific interventions. Interpersonal Communication - The ability to establish rapport quickly and to communicate effectively with consumers, their family members and other providers. Collaboration & Teamwork- The ability to function effectively as a member of an interdisciplinary team that includes behavioral health and primary care providers, consumers and family members. Screening & Assessment- The ability to conduct both brief and comprehensive developmentally appropriate screenings and assessments, and to arrange for more comprehensive assessments when indicated. Care Planning and Care Coordination- The ability to create and implement treatment or rehabilitation plans, and exchange information (with consent) regarding goal establishment and progress with consumers, family members and other providers. Intervention- The ability to provide focused treatment and recovery services, including motivational interventions and crisis intervention. Cultural Competence & Adaptation- The ability to provide services that are relevant to the culture of the consumer and their family. Systems Oriented Practice- The ability to function effectively within the organization and the local Maryland Public Mental Health System. Practice Based learning & Quality Improvement- The ability to assess and continually improve the services delivered as an individual provider and as a member of a multidisciplinary team. Informatics- The ability to use information technology (EMR) to support and improve service delivery. |
