Responsibilities: The Virtual Primary Care Family Medicine Advanced Practice Provider (Nurse Practitioner/Physician Assistant) provides comprehensive primary care to new and established patients for Jefferson Health via telehealth platform. This service will expand Jefferson Health primary care services to new and existing markets. S/he collaborates with, and is an integral member of, a multidisciplinary health care team. Patients may utilize Virtual Primary Care for continued primary care services or as a transition point when waiting for a new or established appointment in their preferred Jefferson primary care location (ldquo;Supplemental Accessrdquo;). The Virtual Primary Care APP will work in conjunction with the population health team to address care gaps and provide equitable, value-based care. Overview: middot; Assessment/Diagnosis and Management of Patients in Primary Care Setting middot; Provides patient-centered, preventive health, including well visits, screenings, and immunizations recommendations; as well as acute and complex care visits requiring diagnosis, treatment/management middot; Collaborates with interdisciplinary health care team. middot; Possesses awareness and provides assessment of patient risk factors, including physical, socioeconomic, and psychosocial risk factors. middot; Obtains and documents thorough patient history. middot; Performs and documents physical exam within the limits of telehealth. middot; Orders, performs, and interprets appropriate diagnostic testing, and responds to results requiring further evaluation or investigation. middot; Formulates differential diagnoses by priority. middot; Engages/Collaborates with collaborative physicians when needed (i.e. diagnosis in question, patient not responding to treatment, etc.). middot; Coordinates with and refers to specialists and/or a higher level of care as needed. middot; Complete professional billing in timely and accurate manner, in compliance with regulations. Formulates Plan of Care middot; Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address preventive care, acute care, and chronic health care needs. middot; Utilizes evidenced-based practice guidelines and protocols in an individualized, dynamic plan of care that can be applied across the continuum. middot; Implements and modifies plan of care based on patient response, diagnostic testing, and specialist recommendations. middot; Prescribes diagnostic strategies and therapeutic interventions (as above). Documentation middot; Telehealth visits are documented in EPIC system using SOAP format. middot; Provides timely documentation in Epic; meets compliance standards; and captures patient history, presenting problem, physical exam within limits of virtual care, diagnosis(es) and treatment/plan of care. middot; Signs/closes visit notes within 24 hours of the patient encounter. Other Responsibilities middot; Reviews results of diagnostic studies in a timely manner, and communicates follow-up plan to the patient middot; Reviews and addresses telephone and Epic portal messages (In-Basket) middot; Reviews specialist consults, recent inpatient summaries, or other patient information copied from other providers middot; Provides In-Basket coverage as needed when other providers are out-of-office Communication and Collaboration middot; Maintains ongoing communication and collaboration with office staff and interdisciplinary health care team. middot; Facilitates and communicates with patients, patient care team, and staff middot; Report compliance concerns or patient safety issues/events. Quality and Productivity middot; Engages with population health team to improve patient access and outcomes. middot; Engages in HCC coding middot; Engages in BPAs to address care gaps, value-based care metrics middot; Aims to meet annual quality metric goals as defined by Jefferson Medical Group. middot; Meets target productivity, measured by work RVUs (relative volume units) Professional Practice middot; Demonstrates Professional Practice behaviors middot; May seek opportunities for active engagement in research and the analysis of evidenced based practice. middot; Completes annual required health system learning activities middot; Attends specialty-specific meetings and encouraged to attend Advanced Practice Grand Rounds (monthly) middot; Maintains national board certification, state licensure and required continuing education, and membership in a professional organization as appropriate middot; Shares telephonic on-call responsibilities with practice team Covid Vaccination is a requirement for employment at Jefferson for employees working at Jeffersonrsquo;s clinical entities or at the University.
